I/we understand that the use of the Credit Card is subject to the Credit Cards Agreement.

The information I/we will give is true and correct to the best of my/our knowledge, and nothing that might affect your decision to approve this application will be withheld.

I/we authorise you to release to, and collect from, any other credit provider, financial institution, consumer or commercial reporting agency or any other party that you, at your sole discretion, determine necessary to evaluate this application, my/our credit information which includes, but is not limited to, information about my/our application for personal and/or business credit, account information, creditworthiness, credit history, credit standing and credit capacity.

I/we have read and accept the terms and conditions of the Credit Cards Agreement and I/we confirm to be bound by such Credit Cards Agreement and any changes that may occur from time to time.
